Please fill out all fields and attach the following items below:

• Contents of Imaging CDs
• Radiology Reports / Procedure Reports (if applicable)

To upload files you can drag and drop all files and folders in the message window below, or follow the steps listed below. Please make sure to send us ALL FILES and FOLDERS from the imaging discs. We need the actual DICOM folder from the disc, not JPEG images.

• Insert the disc (CD) with the imaging studies
• Click on the disc (CD) drive to reveal all disc folders and files
• Click Add Folder button/ Click Upload/ Click Upload/ this adds all folders to que
• Click Add Files button/ Click Upload/ Click Upload/ this adds all files to que
• For medical reports / click add files / select individual reports to add to que
• Click Send
